WELCOME TO THE 2009-2010
MINOR HOCKEY SEASON
Registration Information
KMHA registration will take place at the Sports Palace on Saturday September 19th and Sunday September 20th from 10:00 am to 2:00 pm.

Peewee Players - $ 430.00 $ 150.00 $ 270.00
Bantam Players - $ 450.00 $ 150.00 $ 290.00
Midget Players - $ 435.00 $ 150.00 $ 290.00

New Players
Parents of all new players to Kapuskasing Minor Hockey Association must provide proof of the child’s age (Birth Certificate), and proof of residency. (valid Driver’s License,and a copy of a recent utility bill).
Residential Move
Special forms must be completed for ANY change of address. These forms are available online or from the KMHA office. Please visit the player residential move form or call the office at 335-5774. Along with this form the following documents are required:
1) Official confirmation of school enrolment;
2) Letter to the school from the parents authorizing the OHF to check on and confirm school enrolment during the year;
3) Copy of rental agreement, transfer of ownership or agreement of purchase and sale;
4) A copy of a parent's valid updated Driver’s license;
5) A utility bill containing the new address;
6)Executed copy of any relevant separation agreement, custody order or divorce decree;
7) Letter from parent(s)' employer(s) confirming employment and primary location of that employment
Forms will also be available at registration.
Registration Policy re: Payment
(Cash or cheque payable to Kapuskasing Minor Hockey Association only)
Option 1 – Payment in full at registration
Option 2 – Minimum $ 75.00 per registrant payment at registration and the balance payable by post dated cheques, provide the total owing is paid by December 15, 2009.
Children’s Fitness Tax Credit Receipts
Receipts for the Children’s Fitness Tax Credit will be issued at registration.
Coaching
Coaches are asked to visit our website (www.kapusaksingminorhockey.ca) to get a copy of the coaching application.

Rep Team Tryouts
Tryouts for the KMHA Rep Teams will take place at the Sports Palace on the following days:
Novice Devils Tuesday September 8th 6:00 pm
Thursday September 10th 6:00 pm
Atom Vikings Tuesday September 8th 7:00 pm
Thursday September 10th 7:00 pm
Peewee Angles Wednesday September 9th 6:00 pm
Friday September 11th 6:00 pm
Bantam Ice Hawks Wednesday September 9th 7:00 pm
Friday September 11th 7:00 pm
All players wishing to try out for a Rep Team will be charged a fee of $15.00. Please ensure you are at the arena 1 hour before the try-out time.
Any players selected to play on a Rep Team will be subject to a “Rep Team surcharge”
